If the Payment Specialist recommends it, review and complete the Mortgage Assistance Application, found here.​​
Please include the following items with your completed application:

2 recent paystubs showing year-to-date earnings

A 12-month history from Friend of the Court

Alimony support totals
-
Most recent Social Security benefits letter

Totals of state benefits you receive, such as food assistance or cash assistance

Most recent federal tax return, all pages

Most recent bank statements, all pages

Most recent profit/loss state ment if you are self-employed

Any other sources of income
Once your application is complete, we will review it. You will receive notice in the mail of the outcome or if additional documentation is required.​
